Abstract

The superior shoulder suspensory complex is a ring composed of bony and ligamentous structures, which allows a stable relationship between the upper limb and the axial skeleton. There is a multiplicity of combinations of structures involved in their disruptions. When isolated, lesions of the superior shoulder suspensory complex are generally minimally displaced and can be treated conservatively; however, when a disruption is present at two or more points, its integrity and stability are compromised. In double disruptions, the surgical treatment consists of the reduction and stabilization of at least one of the lesions, and in the triples of at least two. The authors present the case of a patient with a rare glenoid fracture - Ideberg type III - associated with a fracture of the scapula spine and an acromioclavicular dislocation, which occurred after a motorcycle accident. Triple disruptions involving fractures of the scapula spine - as the case we present - are the rarest, with only 3 cases published in the literature. The diagnosis of this lesion was made using conventional radiography and computerized tomography with three-dimensional reconstruction. Treatment consisted of percutaneous glenoid fracture osteosynthesis with cannulated screws, with arthroscopic support, and osteosynthesis of the fracture of the scapula spine with reconstruction plate.
