Extrapulmonary tuberculosis affecting the hand is a rare clinical condition and its diagnosis isn’t easy. It can affect the skin, the subcutaneous and synovial tissue, articulations or even bones, but it’s the synovial tissue the preferred for the bacteria, and the flexor tendons are chosen over the extensors. In the wrist it can simulate a carpal tunnel syndrome but with a different etiology and treatment’s approach. It is known that patients often lie or aren’t aware of its owns past history, so it is very important the physicians suspicion in the presence of any sign of alarm in order the condition to be recognized and properly treated. The authors describe a clinical report of a carpal tunnel syndrome caused by a flexor synovial tuberculosis infection and the signs that should raise suspicion for the proper diagnosis of the condition.
