Objective: Extravasation of contrast material is a rare complication of radiocontrast studies. This article describes a hand compartment syndrome after intravenous injection into the dorsum of hand of contrast agent during the course of a computed tomography. Description: We present a clinical case of a patient of 76 years who developed a left-hand compartment syndrome after intravenous injection of contrast into the dorsum of his left hand while performing a computed tomography. The patient underwent decompressive fasciotomies. Comments: The subcutaneous extravasation of contrast material is a rare complication of imaging studies. The vast majority of extravasations involve small volumes of contrast material. The extravasation of large volumes may result in irreversible damage. The patient should receive detailed and objective information to facilitate immediate recognition of this complication being the conservative treatment effective in most cases.
