Metastatic acetabular disease can be severely painful and may result in loss of ambulatory function. Patients with disease affecting large weight-bearing regions of the acetabulum and with impending or present failure of the hip joint can benefit from surgical treatment. Surgical management of pelvic metastatic disease ranks among the most challenging procedures in orthopedic oncology due to the anatomic complexity of tumor resection and the need to provide immediate stable reconstruction to relief pain and restore ambulatory function. Various reconstructive acetabular techniques have been proposed, mostly with poor functional results and high complications rate. We present a 70 year-old woman with a solitary and symptomatic acetabular metastatic lesion, in which was performed total Enneking zone II and partial zone III resections, followed by pelvic reconstruction with a pedestal cup ball-socket modular prosthesis, and its clinical and functional results.
