Abstract

Background Elastofibroma dorsi (ED) is a rare benign mesenquimatose tumor located in 99% of cases, in the infra or peri-scapular region, between the thoracic wall and the serratus anterior, latissimus dorsi and rhomboid muscles and frequently, adherent to the rib cage periostium. Material and Methods A retrospective study consisting of 6 patients with ED operated on between April of 2009 and February 2014. The clinical presentations were assessed and diagnosis was made based on clinical history, physical examination and on imaging, namely ultrasound and MRI. Surgical treatment consisted of marginal resection of the tumor and histological evaluation. Results All cases presented inferior and peri-scapular swelling. 50% complained of pain with shoulder movement, one with concomitant functional limitation, while another of clunking of the shoulder. Of the remainder, one reported merely a sense of clunking with scapular mobilization. The mean time between the first symptomatic manifestation to surgical treatment was 18 months. The only complications were postoperative - seroma developed in 2 cases, requiring needle aspirations on the 8th and 13th day. Conclusions Given its benign nature and the absence of documented malignant transformation, conservative treatment is indicated in elderly and asymptomatic patients. Surgical treatment is an issue still in debate, although marginal resection is the preferred option. Patients are ruled as cured if the margins of the excised mass are histologically free of tumor. ED should be taken into account in differential diagnosis of shoulder disorders. Given its benignity and mild symptoms, it may be more frequent than stated in the literature.
