Abstract

Objective: Analyze the result of surgical treatment of 9 patients with odontoid fracture with an anterior cervical approach. Material and Methods: Retrospective study of 9 consecutive patients with odontoid fracture, without neurological deficit, submitted to surgical treatment between 2008 and 2013. Fractures were evaluated according to Anderson & D’Alonzo classification. All patients underwent the same postoperative protocol and osteosynthesis, clinical impact, radiologic results and associated complications were analyzed. Results: Mean age was 65.4 years (47-82) and 5 were female. The most common mechanism of fracture was low energy trauma (n=5) and the most frequent pattern of fracture was type II (n=7). All patients were submitted to osteosynthesis with 1 (n=4) or 2 (n=5) cannulated screws with an anterior cervical approach. The mean hospitalization time was 17.2 days (8-23) and mean follow-up was 18.8 (3-65) months. Healing rate was 85,7%, with the occurrence of 1 case of pseudarthrosis (14,3%). Conclusion: The data observed are in line with the literature, with high consolidation rates, regardless of using 1 or 2 cannulated screws, making this osteosynthesis method, a first line therapeutic option for patients with type II odontoid fracture.
