In patients with osteoporotic fractures of the proximal femur, little is known about the incidence and risk factors of contralateral fracture of the proximal femur. The aim of this study was, through a retrospective study, to determine the incidence and risk factors for non-contemporary bilateral fracture of the proximal femur. Clinical files were analyzed from 1911 patients with fracture of the proximal femur between 2003 and 2009. The data collected on the fractures, treatments and comorbidities were processed statistically. A total of 64 patients (3.24%) had bilateral fracture of the proximal femur, with an average age above 80 years. It was determined that there is a direct relationship between the type of the first and second fracture (intracapsular vs extracapsular), and that 70% of second fractures occur in the first three years after the first fracture. Of comorbid conditions it was found that Parkinson's disease, hypertension, heart disease, anemia and abnormal vision represent an increased risk for contralateral fractures of the proximal femur. We propose a more rigorous follow-up in the first three years after the first fracture and established best ways to prevent fractures and optimization of comorbidities in patients with risk factors.
