VITAL, Luísa; ALEGRETE, Nuno  e  LUCAS, Raquel. Correlação clínica e ecográfica em displasia de desenvolvimento da anca. Rev. Port. Ortop. Traum. [online]. 2013, vol.21, n.3, pp.363-370. ISSN 1646-2122.

Purpose: This study aims at relating the clinical findings of the physical examination to the ultrasound results, using the Graf method to classify the dysplasia of the hip. Patients and methods: The clinical and ultrasound records from patients born between January 2010 and December 2012, who were sent to Paediatric Orthopaedics, were retrospectively analysed. The parameters of both the clinical examination and the ultrasound were registered and the results were then compared. Findings: 55 children were identified (46 female children), with a median age of 53 days. Thirty of those children presented at least one risk factor, being the breech presentation at birth the most prevalent one. Among the 110 hips, 80 showed ultrasound changes. Forty-eight hips revealed, at least, one change at the physical examination. The sensitivity and specificity of the ultrasound which has the clinical exam as gold standard was respectively 82,6% and 37,7%. The ultrasound is considered positive when it meets or exceeds Graf’s IIa class. Conclusions: The ultrasound may represent a good screening method; however, it does not confer a significant increase in the diagnostic accuracy of the clinical examination.
