Objective: A hamate fracture is a rare injury corresponding to 2-4% of carpal fractures and has a large percentage of delayed diagnosis. It may reach the body or apophysis (this being the most common). The hamate promotes the stability of the 4 th and 5 th metacarpal and any change in its geometry can result in arthritis or functional impairment. Description: The authors describe a male patient with 3 weeks of development after fracture of the 4th metacarpal base with clinical pain in the ulnar wrist, decreased grip strength and compression of the ulnar nerve. It was found a coronal fracture of the hamate and the patient underwent osteosynthesis with the release of Guyon's canal. The patient was evaluated subjectively and objectively 2 and 4 months after surgery, using criteria such as patient satisfaction, the DASH questionnaire and imaging studies. The patient is satisfied with the result obtained without local pain, grip strength maintained with no clinical cubital compression, having resumed his occupation. Comments: The coronal bone fractures of the hamate are rare and usually occur with fractures of the base of the 4th metacarpal. Whenever there is a fracture of the 4th metacarpal base with shortening of the radius or if the clinic does not match the imaging, should further research be optimized. The fixation of coronal fractures of the hamate is important to obtain carpal stability.
