Abstract

Regional Migratory Osteoporosis (RMO) is a rare clinical syndrome characterized by severe pain on one or more joints. With migratory and sequential nature, predominantly affects load joints in middle-aged men and women in the third trimester of pregnancy. Physical examination may reveal a moderately swollen joint, synovial effusion associated with fl ushing. Limited range of movement is also detected. Load on the joint causes pain, making it diffi cult or even impossible to march. Spontaneous remission of symptoms occurs in about six to nine months, with the maximum reported pain usually around the second month. Recurrence can occur in the same or another joint. The etiology of RMO is unknown. Radiology and bone densitometry typically show transient osteopenia. Changes in MRI imaging include the RMO in the Syndrome of Bone Marrow Edema (SBME). The relationship between the RMO with Transient Osteoporosis of the Hip (TOH), Transient Bone Marrow Edema (TBME) and Osteonecrosis (ON) has been enthusiastically debated. There are several reported cases of Transient Osteoporosis (TO) and RMO. It is believed that the fact that names of RMO, TOH or SBME be assigned to describe the same entity, the true number of reported cases is unknown. A case of a patient that had clinical and imaging features of RMO over a period of two years is reported. It was affected, in a migratory pattern, the two knee joints, with intercondilian migration in both, and a hip joint. For the review was conducted in Pubmed, a systematic search of articles published in Portuguese, English, Spanish and Italian. With the keywords: Regional Osteoporosis, Transient Osteoporosis, Regional Transient Osteoporosis were obtained. 51 articles, of which 29 publications were included, 10 of them systematic reviews. Were included those articles that discuss the diagnosis and treatment of disease, the possible etiologies and pathophysiologic mechanisms involved and correlate the clinical importance of additional diagnostic in orientation and follow-up of this syndrome.
