In the context of the transition from pediatric to adult healthcare, it is critical to assess the self-care and decision-making skills of adolescents and emerging adults with chronic health conditions. Thus, the present study was aimed at examining the psychometric properties of the Portuguese version of the Transition Readiness Assessment Questionnaire (TRAQ), including its factorial structure, reliability, and discriminant validity between sexes, age-groups, and health conditions. The convenience sample for this cross-sectional study comprised 269 portuguese adolescents/emerging adults aged 16 to 26 years, either healthy or with chronic health conditions. The assessment protocol included the TRAQ translated into Portuguese and a clinical and sociodemographic datasheet. After deleting three items because of cultural specificities, the exploratory factor analysis revealed a one-factor structure, instead of the originally proposed five-factor structure. The total scale presented good reliability (Cronbach's alpha = 0.85) and discriminant validity between males and females (t = 4.96; p < 0.001), between adolescents under 18 and emerging adults over 18 years (t = -4.22; p < 0.001), and between healthy and clinical groups (t = 2.01; p = 0.04). These results suggest that the Portuguese version of the TRAQ has adequate psychometric properties regarding reliability and validity, thus representing a valuable tool for use in clinical and research contexts.
