Abstract

Currently bariatric surgery is the most effective treatment for the most severe forms of obesity, to which psychological assessment and intervention are recommended. This study is exploratory, observational, and transversal, with a non-probabilistic sample of 293 subjects (clinical sample of 165 subjects from two Portuguese hospitals and non-clinical sample of 128 college students). In order to assess body image dissatisfaction (BID), we used the Scale of Silhouettes for Obesity Surgery [Escala de Silhuetas para Cirurgia da Obesidade (ESCO)]. The entire clinical sample presented BID (preferred a body with less volume). Approximately 41% of the non-clinical sample was satisfied with the body image and 58.6% presented BID (47.7% preferred a body with less volume and 10.9% a body with higher volume). Bariatric patients presented higher BID (5.15 ± 1.75), than normal-weigh subjects or with pre-obesity (0.54 ± 0.84), t(274) = 29.06, p = 0.001. The difference between participants with different body mass index (BMI) classes, in relation to current body image (CBI) is statistically significative, χ2 (7) = 105.94, p = 0.001, once the subjects from the clinical sample chose images of CBI corresponding to a much higher BMI than their current BMI, in comparison to the subjects from the non-clinical sample. This study suggests that the ESCO, the only instrument designed to assess BID of the Portuguese bariatric patients, contributes to the task of the clinics to design individualized psychological interventions, and to enhance the surgical results.
