The use of psychoactive substances is an important Public health problem that challenges health professionals to rethink the practices oriented to this audience. Aiming to contribute to this issue, this study mapped the therapeutic itineraries of users of psychoactive substances through a qualitative research that describes and analyzes the paths taken in search of attention to this problem in small counties of the State of Rio Grande do Sul/Brazil. The subjects were recruited in a CAPS and subjected to semi-structured interviews, analyzed by the method of Content Analysis. The study results mapped the care network in the region and its difficulties regarding the unsuitable reception of users and the importance of a positive bond with the teams and the role of self-help groups. From the perceptions of interviewees, positives and negatives issues regarding their itineraries were appointed, as well as their relation to the substances used and the need to act as external factors motivating the search for treatment.
