The experience of hospitalization is often a difficult time for the patient, family and staff, which can worsen when there is occurrence of confusion states period. The high incidence of these paintings, especially delirium in hospitalized elderly patients is a challenge to the whole multidisciplinary hospital staff. The care of these becomes exhausting due to difficulties with the prevention, management and treatment, in addition to making even greater hospitalization costs. This research aims to show the difficulties in approaching patients with confusion states and discuss clinical interventions that fit this reality. The survey was conducted from psychological treatment in an orthopedic hospital ward with elderly patients suffering from bone fractures that developed after hospitalization to confusional state. The results suggest that the psychologist’s approach should be given specifically to welcome, support and assist, if possible, patients to resume contact with reality, as seen in Winnicott’s Psychoanalysis and this action cannot be given generically.
