Scales developed to assess pain in adults receiving intensive care are few and insufficiently studied. The aim of this study was the semantic and cultural adaptation of the Behavioral Pain Scale into Portuguese and the assessment of its psychometric properties in patients receiving intensive care, with medical and/or surgical pathology, mechanically ventilated and incapable of self-assessment. This was a methodological study involving translation and assessment of the psychometric validity of the scale, including the organization of panels, in which the scale was applied by two observers simultaneously and independently to 60 patients who had been selected randomly. The process of semantic and cultural validation resulted in a consensus version. Principal component analysis showed one factor with an explained variance between 67,96% and 68,14%. Internal consistency using Cronbach’s alpha was between 0,65 and 0,73. The level of agreement between observers for each item using Kappa values ranged from 0.20 to 0.66. The intraclass correlation coefficient between observations for the total score was 0.79. The Portuguese version of BPS-IP proved to be a valid and reliable instrument if used with adults who are mechanically ventilated and receiving intensive care.
