Cryptogenic Cirrhosis is a diagnosis of exclusion. The prevalence of Obesity and Diabetes Mellitus is higher in patients with cryptogenic cirrhosis, being similar in patients with nonalcoholic esteatohepatitis. Currently, nonalcoholic esteatohepatitis emerges as a possible step preceding the Cryptogenic Cirrhosis. The authors describe the clinical case of a patient obese and diabetic with pancitopenia. It was diagnosed liver cirrhosis, which etiology proved to be a challenge. The results obtained suggest a possible evolution from underlying nonalcoholic esteatohepatitis to cirrhosis. Considering the increasing prevalence of metabolic syndrome, hepatic steatosis and nonalcoholic esteatohepatitis, the risk of progression to cirrhosis must be reduced and education of the population must be promoted.
