Abstract

Stroke-like migraine attacks after radiation therapy (SMART) is an extremely rare syndrome that involves the appearance of complex neurological signs and symptoms or confusional states in patients who have undergone brain radiation years earlier. There are characteristic changes in brain magnetic resonance imaging that normalize with the resolution of symptoms. We report the case of a 64-year-old woman with a history of brain radiotherapy and the onset of inaugural right hemiparesis, confusion and decreased level of consciousness, presenting brain magnetic resonance imaging changes compatible with the diagnosis of stroke-like migraine attacks after radiation therapy. The authors intend to draw attention to the existence of this syndrome and its possible under-diagnosis.
