An 8-month-old male infant, followed in a pediatric consultation for severe atopic dermatitis (AD), went to the emergency department due to exacerbation of AD, anorexia, fever and poor general condition with three days of evolution and without improvement despite being under antibiotic therapy with amoxicillin / clavulanic acid, on suspicion of acute otitis media. On objective examination, multiple chronic eczematous lesions with extensive, exudative and friable erosions were found, predominantly in the perioral region. In addition, he presented multiple target lesions with central vesicle and herpetiform grouping at the shoulders, antecubital and popliteal flexures, back of the hands and perineum, so he was admitted with the diagnostic hypothesis of AD complication in the context of eczema herpeticum and empirically medicated with intravenous amoxicillin / clavulanic acid and acyclovir. The collection of skin exudate confirmed infection with Herpes Simplex Virus type 1. Apyrexia was reached on the third day of acyclovir, with progressive improvement of skin lesions and appetite. Discharged after nine days of hospitalization.
