Introduction: The relationship between eosinophilic esophagitis and allergic disease is increasingly evident, but its relationship with IgA deficiency is unknown. Case report: Seventeen-year-old boy, followed in the Gastroenterology department for recurrent episodes of food impaction. Referred to the Immunoallergology to evaluate sensitization to airborne and food-allergens. Skin prick tests were positive to: shrimp (3 mm), Dermatophagoides pteronyssinus (9 mm) and Alternaria alternata (9 mm). Raised total IgE (692 UI/L) and specific IgE to shrimp (2.16 KU/L), Dermatophagoides pteronyssinus (>100KU/L) and Alternaria alternata (28.2KU/L) and low IgA (<0.07g/L). The paranasal sinuses CT showed marked inflammatory infiltrate. Under therapy with pantoprazol 40mg and degluted fluticasone 250 μg twice a day, with good clinical evolution. Conclusion: This report suggests a possible relationship between eosinophilic esophagitis, allergic disease and the selective IgA deficiency
