The life style or wellness is associated with health and disease. The present study aims to develop a questionnaire to assess health behaviour or lifestyle related with good health. The questionnaire includes 28 items distributed trough five categories, inspired in the 1982 study of Hettler. The items constitute a list of behaviours (rating scale) and not a scale. Participants are a convenience sample of 609 healthy individuals, which fulfil the questionnaire voluntarily, in an anonymous and confidential way. The study of the questionnaire as a measure show that it constitutes a brief measure of health related behaviour useful to be used in research and programmes of health promotion and disease prevention.
